By submitting this form, I authorize CCT Repairs to:

1. Inspect and diagnose my device upon receipt.
2. Contact me with a repair estimate before performing any work.
3. A diagnostic fee applies: $34.99 (phones), $44.99 (computers), $54.99 (other devices), plus CT sales tax. This fee is applied toward the total repair cost if approved.
4. CCT Repairs is not responsible for data loss. Please back up your device before shipping.
5. Devices not picked up or paid for within 60 days may be recycled.
6. Return shipping costs will be added to the final invoice.

All repairs come with a 90-day limited warranty.
